This is an evaluation using psychological testing administered by a fully qualified and licensed staff to help determine your diagnosis in order to recommend the best treatment for you.

All Other Assessments and Testing
This refers to professional assessments and testing, other than psychological tests, that are done to determine your level of functioning and your treatment needs.

Health Assessment
This includes activities provided by a physician, registered nurse, physician assistant, nurse practitioner, or dietitian to determine your need for services and to recommend specific treatment.

Evaluations for Physical / Occupational Therapy or Speech, Hearing or Language, Therapy
Used to determine the beneficiary’s need for services to prevent, treat, or alleviate physical or mental disability.
